What is the MRCEM Exam?
The Membership of the Royal College of Emergency Medicine (MRCEM) is the definitive postgraduate qualification for doctors pursuing a career in Emergency Medicine in the United Kingdom. It is awarded by the Royal College of Emergency Medicine (RCEM) and recognised by the General Medical Council (GMC) as a marker of clinical competence and knowledge in acute and emergency care.
Passing the MRCEM is a mandatory step in the UK emergency medicine training pathway. It demonstrates your mastery of the RCEM curriculum — spanning basic sciences, clinical reasoning, emergency presentations, and procedural skills — and is recognised internationally, with exams held in the UK, Malaysia, and India.
The MRCEM is the Membership examination and acts as an intermediate qualification during core training. The FRCEM (Fellowship) is the higher examination required for completion of specialist training. You must pass MRCEM before sitting FRCEM.
The MRCEM consists of three sequential components — the Primary, the Intermediate SBA, and the OSCE — all of which must be passed in order to achieve full MRCEM status and become eligible for College Membership.
MRCEM Exam Structure & Duration
The MRCEM examination programme uses validated, evidence-based assessment methods to test knowledge, clinical skills, and professional behaviours across all three components. Here is a detailed breakdown:
MRCEM Primary
MRCEM Intermediate SBA
MRCEM OSCE
You must pass MRCEM Primary before sitting the Intermediate SBA, and pass the Intermediate SBA before you are eligible for the OSCE. There are no shortcuts.
MRCEM Eligibility Criteria
Eligibility requirements differ slightly between each component of the MRCEM. Below is a complete summary of who can apply at each stage.
MRCEM Primary — Eligibility
- Hold a medical qualification approved by the GMC for the purposes of registration.
- Candidates are strongly advised to have an IELTS score of Level 7 or equivalent English proficiency before applying.
- Must be familiar with the RCEM Basic Sciences Curriculum (June 2010).
MRCEM Intermediate SBA — Eligibility
A candidate is eligible to sit the MRCEM Intermediate SBA if they meet all of the following:
-
aGMC-approved Medical Qualification
Hold a medical degree approved by the General Medical Council for the purposes of registration in the UK.
-
bUK Foundation Programme (or equivalent)
Have completed the UK Foundation Programme or an equivalent programme. Electronic evidence of completion will be required at the point of application.
-
cPassed the MRCEM Primary (or predecessor)
Must have passed one of: MRCEM Part A (after 1 Aug 2012, before 31 Jul 2016); FRCEM Primary (after 1 Aug 2016); or MRCEM Primary (after 1 Aug 2021). DREEM trainees with MRCS/MRCSI exemptions are also eligible.
If you qualified outside the UK, you will need to demonstrate that your primary medical degree is on the GMC's approved list. IMG doctors should also check IELTS / OET requirements carefully before applying.
MRCEM OSCE — Eligibility
To be eligible for the MRCEM OSCE, you must have successfully passed the MRCEM Intermediate SBA examination. The OSCE tests clinical and communication skills and is the final hurdle before achieving full MRCEM status.
MRCEM Syllabus & Exam Content
Each component of the MRCEM is mapped to a different part of the RCEM Emergency Medicine 2021 Curriculum. Here is what you need to know for each paper.
MRCEM Primary — Syllabus
The Primary is based on the RCEM Basic Sciences Curriculum (June 2010), which covers the fundamental scientific principles underpinning emergency medicine. The MRCEM Primary was previously called the FRCEM Primary (August 2016 – July 2021).
Anatomy
Applied anatomy relevant to emergency procedures and presentations
Physiology
Cardiovascular, respiratory, renal, neurological and endocrine physiology
Pharmacology
Drug mechanisms, pharmacokinetics, and pharmacodynamics relevant to the ED
Microbiology
Common pathogens, infection control, and antimicrobial principles
Pathology
Disease mechanisms and pathophysiology seen in the emergency department
Evidence-Based Medicine
Statistics, research methodology, critical appraisal of trials
MRCEM Intermediate SBA — Syllabus
The Intermediate SBA covers the full range of conditions and presentations that may be encountered in an Emergency Department. This is a broad, clinically-focused paper requiring strong application of knowledge to real scenarios. Key topic areas include:
Cardiology & Resuscitation
ACS, arrhythmias, cardiac arrest, ACLS protocols
Neurology & Neurosurgery
Stroke, seizures, headache, raised ICP, spinal emergencies
Respiratory Medicine
Asthma, COPD, PE, pneumothorax, respiratory failure
Trauma & Surgery
Major trauma, ATLS principles, acute surgical abdomen
Paediatric Emergencies

MRCEM OSCE — Syllabus
The OSCE assesses clinical and communication skills in a simulated emergency environment. Stations may include procedural skills (airway management, resuscitation, joint aspiration), history-taking, clinical examination, and breaking bad news or consent discussions.

How to Pass the MRCEM: Proven Strategy
Passing the MRCEM demands consistent, structured revision over several months. With a national pass rate that can be challenging — particularly for the Intermediate SBA and OSCE — a strategic approach to preparation makes a significant difference.
1. Start Early and Make a Revision Plan
Most successful candidates begin structured revision 3–6 months before their exam date. Map the syllabus topics to your calendar, ensuring every area receives adequate coverage. Avoid cramming — spaced revision across weeks is far more effective for long-term retention.
2. Practice Questions from Day One
The MRCEM SBA format rewards pattern recognition and clinical reasoning — skills that are built through repeated practice with high-quality MCQs, not passive reading alone. Aim to answer at least 50–100 questions per revision session, reviewing every wrong answer in detail.
3. Focus on Real Exam Recalls
Questions from recent exam sittings (recalls) are invaluable for understanding the style, difficulty, and topic distribution of the actual exam. Platforms like ReviseMRCEM maintain an actively updated database of 500+ authentic exam recalls submitted by recent candidates.
4. Use Clinical Images for the OSCE
For the OSCE and image-based SBA questions, practise interpreting X-rays, ECGs, CT scans, and MRIs regularly. Being able to quickly identify key findings under exam pressure is a skill built over time.
5. Track Your Weak Areas and Improve
Use data from your practice sessions to identify your consistently weakest topics. Allocate extra revision time to these areas. AI-powered tools can automate this process, surfacing weak topics and scheduling targeted review sessions for you.
6. Simulate Exam Conditions with Mock Papers
In the final weeks before your exam, complete full-length mock papers under timed conditions. This builds exam stamina, improves time management, and reduces anxiety on exam day.
